This resource explores opportunities for successful relationships across housing and adult social care services. It covers a range of practice areas in which these services may overlap, including support for older and/or disabled people and adults facing homelessness who may have care and support needs. The resource proposes tips, tools and examples for effective partnership working, drawn from practice and research evidence.

Families and homes: Change Project
The Families and Homes Change Project brought together academics, professionals from housing, children and families and adults social care, and people with lived experience. They worked together to develop tools to enable better support for families at the intersection of social care and housing.
This July 2023 version is the latest update of the original, first published in December 2019.
